[Graduation Clearance Notice] Printed theses submitted to the Office of Academic Affairs must include the National Central Library’s Application for Embargo of Thesis/Dissertation.

Pursuant to the announcement issued on December 27, 2022, entitled “Important Notes on Graduation Clearance and Degree Certificate Collection for the First Semester of the 2022 Academic Year,” graduate students completing graduation clearance procedures who submit printed theses with delayed public release to the Office of Academic Affairs are required to additionally submit the Application for Embargo of Theses and Dissertations issued by the National Central Library.

The application form must bear the signatures of the student and the thesis advisor and be stamped by the departmental reviewing authority. Relevant supporting documentation for the delayed public release must also be enclosed.

All documents shall be inserted (and not bound into the thesis) and submitted together with the printed thesis to the Registration and Curriculum Division, Office of Academic Affairs, for graduation clearance processing.
